The practical answer

Mortgage guidelines are detailed and change over time, so the correct answer is always scenario-specific. The useful question is not “Can this ever be done?” but “What is allowed for this borrower, property, program and lender today?”

What I look at first

Loan program and occupancy. Agency, government, renovation and Non-QM rules differ.
Property and transaction details. Value, condition, contract terms and timing can change the solution.
Documentation. The best structure still has to be supported and underwritten.

Can an appraisal be challenged?

A lender may have a reconsideration-of-value process when there is credible, relevant information that may not have been considered. The process and acceptable evidence depend on the lender and appraisal rules.

Does the buyer have to pay the difference?

Not necessarily. Possible outcomes can include renegotiation, additional cash, a reconsideration process, changing financing structure or terminating if permitted by the contract.

Can the lender just order a new appraisal?

Not simply because the value is disliked. Appraisal independence rules and lender policies control when another appraisal may be appropriate.
